At a time when freshmen were not eligible, Matthews made his mark as a two-year starter in goal for the Blue Jay lacrosse team. He earned First Team All-America honors as a junior and senior, when he led Johns Hopkins to back-to-back runner-up finishes in the NCAA Tournament (both title-game losses came by just a goal). He also earned the C. Markland Kelly Award as the nation's top goalie in each of those seasons as well and finished his career ranked second on JHU's all-time saves list (320).

Matthews was named to the All-Time Johns Hopkins Team at the end of his career and was inducted into the National Lacrosse Hall of Fame in 1996.

In football Matthews played wing-back and defensive back for the Blue Jays and helped the team compile a 17-10 overall record. The 1971 and 1972 teams both posted 6-3 records with the six wins just one off the school-record for wins in a season at the time. Matthews returned both kickoffs and punts in addition to his duties as a member of JHU's secondary. Complete defensive statistics are not available for Matthews' last two years, but he did lead the team in kickoff and punt returns as a sophomore.

In addition to his exploits as a student-athlete at Hopkins, Matthews continued his service to the University, serving as one of the team doctors for the Department of Athletics.
